PROSPECTIVE STUDENTS ADMITTED TO THE INDEPENDENT PATHWAY OF THE DIPLOMA PROGRAM

ACADEMIC YEAR 2019/2020
It is announced to all participants of the New Student Admission Selection of Institut Seni Indonesia Yogyakarta through the Diploma Program Independent Selection Pathway, that the names mentioned below are declared accepted as prospective new students of ISI Yogyakarta for the 2019/2020 academic year.
Furthermore, prospective students who are declared accepted are required to carry out registration by fulfilling the following provisions and requirements:
1.Download the Terms of Registration and read it carefully on the page www.pmb.isi.ac.id information column.
2. Pay off the fees Single tuition fee (UKT) according to what has been determined based on the UKT tariff group criteria and the choice of study program, and an Institutional Development Contribution (SPI) of Rp. 5,000,000.00. payable from May 22 - 27, 2019through all BNI Bank counters by mentioning the ISI Yogyakarta College Code. (8060) followed by billing number (ISI Yk Diploma Program Independent Pathway Test number) 9-10 digits
3. Fill out a statement form to become an ISI Yogyakarta student, according to the Diploma 3 / Diploma 4 level, signed on a stamp and photocopied in triplicate (forms can be downloaded at www.pmb.isi.ac.id information column).
4. Fill out and sign the draft Study Plan Card (KRS) according to the selected study program (the form can be downloaded at www.pmb.isi.ac.id information column).
5. Fill out and sign the BNI ATM Application Form (form can be downloaded at www.pmb.isi.ac.id information column).
6. Print out the registration flow check-list (form can be downloaded at www.pmb.isi.ac.id information column).
7. MANDATORY Attendance and may not be represented on Monday, May 27, 2019 at 08.00 - 14.00 WIB at the ISI Yogyakarta Rectorate Building to carry out re-registration / registration by submitting:
- Photo copy of the Diploma Program Independent Pathway Participant Card as much as 1 (one) sheet.
- Copy of proof slip of payment of UKT and SPI education fees (item 2 above) from Bank BNI as much as 2 (two) sheets.
- Student Biodata and NIM (2 copies) that have been signed (obtained during registration at counter 1).
- Certificate of Participating in the National Examination from school / Photo copy of National Examination Card / Photo copy of Certificate of Graduation (for 2019 graduates) as much as 1 (one) sheet.
- Photo copy of legalized diploma and SKHUN (for 2018 and previous graduates) as much as 1 (one) sheet.
- Photo copy of KTP / Family Card as much as 1 (one) sheet.
- Photo copy of Birth Certificate as much as 1 (one) sheet.
- Statement of Being a Student of ISI Yogyakarta (Original stamped Rp 6,000, - signed) and photocopied in triplicate.
- Drug Free Certificate and Health Certificate from a doctor according to the provisions of each faculty.
- Recent color passport with red background, size 3×4 as many as 4 sheets. (official passport with neat clothes not wearing a t-shirt) put in a small transparent plastic bag.
- 1 (one) Rp 6,000.00 stamp duty to be signed in the BNI account opening application form.
- Pay for student insurance with a fee of IDR 100,000.00 (One hundred thousand rupiah).
- Candidates who withdraw cannot withdraw the fees paid.
- Complete files (points a-n) are arranged in order, put into a color folder according to the faculty (FSP; red, FSR; blue, FSMR; yellow).
8. Prospective students who are not present until 14.00 WIB are declared resign and lose their rights as ISI Yogyakarta students for the 2019/2020 academic year.
Beware of fraud attempts on behalf of the PMB Committee. The committee does not contact personally / by telephone to all prospective students who have passed the selection.
