\nREGISTRATION OF NEW INDEPENDENT STUDENT CANDIDATES

\nACADEMIC YEAR 2019\/2020<\/h4>\n

  1. Download and fill out the Terms of registration (form can be downloaded on the page pmb.isi.ac.id<\/a> information column.<\/li>\n
  2. Pay off fees Institutional Development Contribution (IDC) of Rp. 5 million rupiah <\/strong>and Single tuition fee (UKT)<\/strong> based on the UKT tariff group criteria and the choice of study program, payable from the 30th<\/strong> July - August 5, 2019<\/strong>, through all BNI National Bank Teller Mentioning the College Code (PT) (ISI Yk. <\/strong>PT Code 8060) <\/strong>Next, the billing number uses the ISI Yk Independent Pathway Test number. 10-11 digits Example: 8060 1932002XXX<\/strong><\/li>\n
  3. Fill out a statement form to become an ISI Yogyakarta student, in accordance with the Regular path, signed on a stamp and photocopied in triplicate (forms can be downloaded at pmb.isi.ac.id<\/a> information column).<\/li>\n
  4. Fill out and sign the draft Study Plan Card (KRS) according to the selected study program (the form can be downloaded at pmb.isi.ac.id<\/a> information column).<\/li>\n
  5. Fill out and sign the BNI ATM Application Form (form can be downloaded at pmb.isi.ac.id<\/a> information column).<\/li>\n
  6. MANDATORY ATTENDANCE <\/strong>and may not be represented on the day<\/strong> Tuesday<\/strong> 6th<\/strong> August 2019 at 08.00 - 14.00 WIB <\/strong>at the ISI Yogyakarta Rectorate Building to carry out re-registration \/ registration of file submission by submit<\/u>:\n
      \n
    1. Photo copy of 2019 Independent Pathway Participant Card as much as 1 (one) sheet.<\/li>\n
    2. Copy of proof slip of payment of tuition fees\/UKT and SPI (item 2 above) from Bank BNI as many as 2<\/strong> (<\/strong>two<\/strong>) sheet<\/strong>.<\/li>\n
    3. Biodata and Student NIM (4 copies) that have been signed (obtained during registration at counter 1)<\/li>\n
    4. Legalized copies of diplomas and SKHUN for 2018 and previous graduates, legalized copies of SKL \/ SKHUS \/ SKHUN for 2019 graduates, 1 sheet each.<\/li>\n
    5. Statement Letter as ISI Yogyakarta Student (Original stamped Rp 6,000, - signed) and photocopied in triplicate.<\/li>\n
    6. Drug Free Certificate and Health Certificate from Doctor<\/li>\n
    7. Latest color passport with red background, size 3\u00d74 as many as 2 sheets. (official photo with neat clothes not wearing a t-shirt) put in a small transparent plastic bag.<\/li>\n
    8. 1 (one) Rp 6,000.00 stamp duty to be signed in the BNI account opening application form.<\/li>\n
    9. Pay for student insurance with a fee of IDR 100,000.00 (One hundred thousand rupiah)<\/li>\n
    10. Prospective students who have not registered on August 6, 2019, until 16.00 WIB are considered to have resigned and lost their right to become ISI Yogyakarta students.<\/li>\n
    11. Candidates who withdraw cannot withdraw the fees paid.<\/li>\n
    12. Complete files (points a-h) are arranged in order, put into color folders according to the faculty (FSP; red, FSR; blue, FSMR; yellow)<\/li>\n<\/ol>\n<\/li>\n
    13. Beware of fraud attempts on behalf of the PMB Committee.<\/strong> The committee does not contact personally \/ by telephone to all prospective students who have passed the selection.<\/li>\n<\/ol>\n
