PT Code 8060) <\/strong>Next<strong>billing number using <\/strong>ISI Yk SNMPTN Registration no. 10 digits (exs:<strong> 1924110xxx)<\/strong> ,<strong>Example: 8060 1924110xxx<\/strong><\/li><li>Fill out a statement form to become an ISI Yogyakarta student, in accordance with the Regular \/ Bidikmisi path, signed on a stamp and photocopied in triplicate (forms can be downloaded at <a href=\"http:\/\/www.pmb.isi.ac.id\">www.pmb.isi.ac.id<\/a> information column). <\/li><li>Fill out and sign the draft Study Plan Card (KRS) according to the selected study program (the form can be downloaded at <a href=\"http:\/\/www.pmb.isi.ac.id\">www.pmb.isi.ac.id<\/a> information column). <\/li><li>Fill out and sign the BNI ATM Application Form (form can be downloaded at <a href=\"http:\/\/www.pmb.isi.ac.id\">www.pmb.isi.ac.id<\/a> information column).<\/li><li>Print out the registration flow check-list (form can be downloaded at <a href=\"http:\/\/www.pmb.isi.ac.id\">www.pmb.isi.ac.id<\/a> information column). <\/li><li><strong>MANDATORY ATTENDANCE and may not be represented on Monday April 15, 2019 at 08.00 - 14.00 WIB <\/strong>at the ISI Yogyakarta Rectorate Building to carry out re-registration \/ registration by submitting:<ul><li>Photo copy of SNMPTN 2019 Participant Card as much as 1 (one) sheet.<\/li><li>Photocopy of proof slip of payment of tuition fees\/UKT (item 2 above) from Bank BNI as many as <strong>2 (two) sheets<\/strong>. For those who are accepted by the <strong>Bidikmisi special <\/strong>do not need to make UKT payments at Bank BNI, UKT payments will be taken into account in receiving the 2019 Bidikmisi scholarship.<\/li><li>Biodata and Student NIM (2 copies) that have been signed (obtained during registration at counter 1)<\/li><li>Certificate of Taking the Exam \/ Photocopy of Exam Card as much as 1 (one) sheet.<\/li><li>Statement Letter as ISI Yogyakarta Student (Original stamped Rp 6,000, - signed) and photocopied in triplicate<\/li><li>Drug Free Certificate from Doctor in accordance with the provisions of each faculty<\/li><li>Recent color passport with red background, size 3\u00d74 as many as 4 sheets.
